Job Title

A compassionate Case Manager is sought to oversee and coordinate patient care within our Long-Term Care facility.

Key Responsibilities:
  • Coordinate individualized care plans for residents based on comprehensive assessments and clinical goals.
  • Conduct initial and ongoing assessments of residents' medical, functional, and psychosocial status.
  • Serve as a liaison between residents, families, physicians, nursing staff, therapists, and community services.
  • Ensure care plans are implemented effectively and adjusted as residents' conditions evolve.
  • Advocate for residents' rights, preferences, and best interests throughout the care continuum.
  • Facilitate timely transitions between care levels (e.g., hospital admissions, home care, palliative services).
  • Monitor compliance with quality standards and facility policies.
Qualifications:
  • Bachelor's Degree in Nursing, Social Work, or a related healthcare field.
  • Current license from a recognized health regulatory authority.
  • Minimum 5 years of clinical experience, with at least 2 years in a case management or long-term care setting.
  • Certification in Case Management is an asset.
  • Knowledge of geriatric care, rehabilitation, chronic disease management, and end-of-life care.
